Employees: Form 1095-C is a tax document that provides information about the health insurance coverage that was offered to you during the year. It is not required to be submitted with your federal tax return.
We are not required to automatically distribute Form 1095-C to employees. However, you may request a copy at any time. If you would like to receive a copy of your Form 1095-C, please contact Human Resources. In accordance with federal regulations, they will provide your requested form within 30 days of your request.
